I am not paneled with any insurance providers. I can provide you with a Superbill if you would like to submit to your Insurance for reimbursement. See below for more information.
A Superbill is an itemized list of all services provided to a client. It means the client pays out-of-pocket and the clinician provides a receipt/“Superbill” that can be submitted to insurance for partial reimbursement.
The Superbill will show the dates of treatment, the treatment code (individual, couples, or family therapy), length of appointment, diagnosis (major depressive disorder, for example), and the amount you paid. Our office will provide you with a Superbill once a month.
Please keep in mind that insurance companies may or may not request the clinician’s records in order to verify treatment. As a practice that is not paneled with any insurance companies, Ari may or may not keep records according to the requirements set forth by insurance organizations. This may or may not affect your coverage.

What is the reimbursement rate?
How many days does one have from the Service Date (appointments) to submit for reimbursement? Every insurance is different, usually 90-160 days.
Minimum number of sessions/or dollar amount you have to spend on treatment before you can submit a receipt/”Superbill”.
Maximum number of sessions/or dollar amount they will reimburse.
How to submit your Superbill (mail in, email, fax, etc.). Typically one mails in the Superbill to the address on the back of the Insurance card.
Are there any limitations to the type of clinician licensure (licensed marriage and family therapist vs. licensed clinical social worker vs. psychologist, etc.) Ari is an LMFT (licensed marriage and family therapist).
Are there any limitations based on type of treatment (cognitive behavior therapy, narrative therapy, EMDR, etc.)? Ari utilizes “Cognitive Behavioral Therapy (CBT)” and “Narrative Therapy”.
Is there anything else I need to know about limitations or the reimbursement process in general?
